2008年门、急诊抗菌药物合理应用调查分析

Analysis of Utilization of Antibacterials in the Outpatient and Emergency Department in 2008

目的:了解医院门、急诊抗菌药物使用情况,以便及时发现存在问题,促进合理用药水平的提高。方法:随机抽取我院2008年门、急诊西药处方12550张,统计各科室抗菌药物使用率、联用率、用药频度(DDDs)等指标,并进行合理性分析。结果:门诊抗菌药物使用率41.92%,单一用药占76.64%,二联及以上联用率23.36%。注射抗菌药物处方占抗菌药物处方的41.46%,口服抗菌药物使用前3名的是头孢克洛缓释片、阿奇霉素分散片和克拉霉素胶囊;注射抗菌药物最多的是头孢美唑;外用主要是氟喹诺酮类。结论:我院门、急诊抗菌药物使用基本合理,但仍存在一定的问题,医院仍需进一步加强抗菌药物合理使用的管理,相关部门应通过采取处方点评等干预措施,以增强合理用药意识,确保用药安全合理。

OBJECTIVE ： To investigate the utilization of antibacterials in the emergency and out - patient department in our hospital so as to find out the problem and improve the rational level of drug use. METHODS： 12 550 prescriptions of western medicines were randomly sampled from the emergency and out - patient department in our hospital in 2008 for a statistical analysis of antibacterials regarding the indexes such as the proportion of patients using antibacterials, rate of drug combination, DDDs etc and the rationality of using antibacterials. RESULTS： The rate of using antibacterials was 41.92% in the outpatients. The rate of using only one kind of antibacterials was 76.64 % versus 23.36 % of using two or more than two kinds of antibacterials. Prescriptions of antibiotic injections represented 41.46% in the prescriptions of antibacterials. The top 3 oral antibacterials were Cefaclor Sustained - release Tablets, Azithromycin Dispersible Tablets and Clarithromycin Capsules. Cefmetazole took the lead among all the antibiotic injections. Quinolones took the lead among all the antibacteirals for external use. CONCLUSION： The utilization of antibacterials in the emergency and out - patient department in our hospital was rational on the whole, but there were irrational prescriptions. The management on the rational use of antibacterials should be further strengthened in hospital and the intervention measure such as evaluation on prescriptions should be taken by concerned department to and increase the consciousness on rational use of drugs and ensure safe and rational medication.
