我院神经外科抗菌药物使用调查分析

Analysis of Utilization of Antibiotics in the Neurosurgery Patients of Our Hospital

目的:调查我院神经外科抗菌药物使用情况,并进行合理性分析,以提高合理用药水平。方法:随机抽取我院神经外科2006年6月～12月每月任意连续7日出院患者病历共156份,根据《抗菌药物临床应用指导原则》对抗菌药物应用进行评价。结果:156例病历中,应用抗菌药物的病历为143例。抗菌药物的使用率为91.67%。其中,手术病历54例;平均手术时间3.5h,抗菌药物平均使用时间为14d。非手术病历89例;抗菌药物使用平均时间为7.8d。90.5%的病历抗菌药物的使用不合理。使用频率较高的抗菌药物是第2、3代头孢菌素、氟喹诺酮类、青霉素类。结论:我院神经外科抗菌药物的应用存在很多不合理的现象,用药时间长、手术预防用药选用档次高、换药频繁及病原学检查不够重视是其主要特点。

OBJECTIVE： To investigate the utilization of antibiotics in neurosurgery patients of our hospital and to analyze the rationality of drug use so as to improve the rational drug use level. METHODS： A total of 156 medical histories of patients who had been discharged from the neurosurgery department randomly sampled in 7 consecutive days each month from June to Dec in 2006 were evaluated regarding the utilization of antibiotics in accordance with the ＂Guiding principle of the clinical application of antibacterials＂. RESULTS： Of the 156 cases, 143 （91.67） were treated with antibiotics, of which, 54 underwent surgery. The average operation time lasted 3.5 hours and the average duration for patients receiving antibiotics was 14 days; 89 were non - surgery cases, their average duration of receiving antibiotics was 7.8 days. Irrational use of antibiotics was found in 90.5 % of the medical records. The second and the third generations cephalosporins, quinolones and penicillins took the lead on the list of DDDs. CONCLUSION： The utilization of antibiotics in the neurosurgery patients in our hospital was far from rational and which was characterized by prolonged medication duration, high level of drugs in preventive medication, frequent change of drugs and lack of attention to the etiology test etc.
