摘要
目的:检测沙眼衣原体和支原体等泌尿生殖系统感染的男性不育患者精液标本中精子DNA断裂情况,并分析抗生素治疗对其影响。方法:使用精子染色质分散试验分别检测143位沙眼衣原体和支原体等泌尿生殖系统感染男性不育患者和50位有生育能力的健康对照者的精子DNA断裂情况,并对其经典的精液常规参数(精子浓度、形态学、活力和活率)进行分析,同时,将部分患者分为抗生素治疗组和非治疗组分析抗生素治疗对精液DNA断裂情况和精液常规参数的影响,并结合临床结果分析检测意义。结果:沙眼衣原体和支原体等泌尿生殖系统感染对精子DNA完整性的影响明显高于其对经典精液常规参数的影响。感染可导致男性生育能力的下降,抗生素治疗可提高妊娠成功率,并可降低感染诱导的精子DNA断裂水平。结论:与精液常规参数相比,精子DNA断裂水平检测可更好地评估男性沙眼衣原体和支原体等泌尿生殖系统感染不育患者的生育能力。同时,DNA完整性可作为一个有用的参数监控治疗的有效性。
Objective:To determine the frequency of sperm cells with fragmented DNA in semen samples from men with genitourinary infection by Chlamydia trachomatis and Mycoplasma and to analyze the influence of antibiotic therapy on it. Methods: The sperm chromatin dispersion test was employed to detect the sperm DNA fragmentation in 143 infertile men with genitourinary infection by Chlamydia trachomatis and Mycoplasma and 50 healthy fertile subjects. At the same time, the classical semen parameters, therincluding sperm concentration, motility, vitality and morphology were evaluated. Furthermore, the impact of antibiotic therapy on sperm DNA fragmentation and all the classical semen parameters were analyzed. At last, the significance of sperm DNA fragmentation detection was explored after being combined with the clinical outcomes. Results:Infection of Chlamydia trachomatis and Mycoplasma could lead to decreased pregnancy rates and showed a more significant impact on sperm DNA damage, than on the injury to other seminal parameters. Antibiotic therapy was successful in decreasing the percentage of spermatozoa with fragmented DNA and was found to improve pregnancy rates. Conclusion: Sperm DNA fragmentation detection could be employed as a much better parameter to evaluate the potentia generandi of the infertile men with genitourinary infection by Chlamydia trachomatis and Mycoplasma. In addition, it could be a useful parameter to monitor the effectiveness of antibiotic therapy.
