摘要
目的探讨儿童急性白血病(acute leukemia,AL)血浆基质细胞衍生因子-l(stromal cell derivedfactor-1,SDF-1)的水平及临床意义。方法采用酶联免疫吸附法(enzyme-linked immunosorbent assay,ELISA)分别检测43例初诊未治 AL 患儿(初诊组)、41例完全缓解 AL 患儿(缓解组)及20例健康儿童和10例非恶性血液病患儿(对照组)的血浆 SDF-1水平。结果①初诊组和缓解组 SDF-1水平明显高于对照组(P<0.01),且初诊组高于缓解组(P<0.01)。②初诊组中急性淋巴细胞白血病(acute lymphocytic leukemia.ALL)患儿 SDF-1水平明显高于急性髓性白血病(acute myelocytic leukemia,AML)(P<0.01)。而 SDF-1水平在 ALL 各亚型组(L_1、L_2、T-ALL 与 B-ALL)和 AML 各亚型组(M_2、M_3、M_1和 M_5)分别比较,差异无统计学意义(P>0.05)。③SDF-1水平与初诊时外周血白血病细胞数量呈正相关关系(r=0.57,P<0.05)。结论①白血病患儿初诊时血浆 SDF-1呈高水平表达,提示它可以作为 AL 的一种检测指标。②SDF-1水平在 ALL 明显高于 AML,说明其与 AL 的类型具有一定相关性。③SDF-1在 AL 患儿初诊时为高水平表达,在完全缓解后表达水平下降,复发时其表达水平又升高,提示动态检测 SDF-1水平,有助于判断病情发展。
Objective To study the level of plasma stromal cell derived factor-l(SDF-1) and clinical significance in childhood acute leukemia. Methods The plasma SDF-1 was measured by enzyme-linked diagnosed (new 20 healthy chi Results ① The mmunosorbent assay (EIASA) in 43 children acute leukemia who were newly y diagnosed groups) ,41 complete remission patients(complete remission groups), dren and 10 children of non malignant hematological disease(control group). level of SDF-1 in newly diagnosed groups of acute leukemia and complete remission groups were significantly higher than that in control groups ( P 〈0.01). And the level of SDF-1 in newly diagnosed patients was significantly higher than that in complete remission patients. ②The level of SDF-1 in acute lymphocytic leukemia(AM.) groups was significantly higher than that in acute myelocytic leukemia(AML) groups( P 〈0.01). The levels of SDF-1 in the subtypes of ALL. (L1 and L2,T-ALL and B-ALL) and AML (M2,M3,M4 and M5) were no significant difference( P 〉0.05). ③The level of SDF-1 was positively correlated with the number of leukemia cells in peripheral blood at the diagnosis time ( r =0.57, P 〈0.05). Conclusion ① High level of SDF-1 in childhood acute leukemia at diagnosis could be considered as a detective index. ②The level of SDF 1 in ALL was higher than that in AML,which indicated that SDF-1 was correlated with the classification of acute leukemia. ③High level of SDF-1 was detected from the patients with acute leukemia at diagnosis time, decreased at remission, increased again at relapse. So monitoring the level of SDF-1 was helpful to judge the developing trends of acute leukemia.
