背景:CD4^+CD25^+T细胞是一种免疫调节细胞,CD4^+CD25^+T/CD4^+T比值在免疫调节中起主要作用,并维持机体的免疫平衡。目的:探讨双歧杆菌、乳杆菌、嗜热链球菌三联活菌片对缓解期溃疡性结肠炎(UC)患者CD4^+CD25^+T细胞的影响。方法:33例缓解期UC患者分为UC对照组和UC治疗组,后者予双歧杆菌、乳杆菌、嗜热链球菌三联活菌片,并设正常对照组。治疗后行症状和内镜评分,以流式细胞仪检测外周血单个核细胞(PBMC)和肠黏膜中CD4^+CD25^+T细胞和CD4^+T细胞比例,以实时定量逆转录聚合酶链反应(RT-PCR)检测PBMC和肠黏膜中Foxp3 mRNA含量。结果:UC对照组症状和内镜评分与UC治疗组相比均无明显差异。与正常对照组相比,UC对照组患者PBMC中CD4^+CD25^+T/CD4^+T比值显著增高(P〈0.05),肠黏膜中CD4^+CD25^+T/CD4^+T比值显著降低(P〈0.05)。与UC对照组相比,UC治疗组患者PBMC中CD4^+CD25^+T/CD4^+T比值显著降低(P〈0.05),肠黏膜中CD4^+CD25^+T/CD4^+T比值显著增高(P〈0.05),与正常对照组无明显差异。三组间PBMC和肠黏膜中Foxp3 mRNA含量无明显差异。结论:缓解期UC患者CD4^+CD25^+T/CD4^+T比值发生改变,表明免疫调节功能出现异常。双歧杆菌、乳杆菌、嗜热链球菌三联活菌片具有免疫调节作用,可使CD4^+CD25^+T/CD4^+T比值恢复正常。
Background: CD4^+ CD25^+ T cell is a kind of immunoregulatory cells. CD4^+ CD25^+ T/CD4^+ T ratio plays a critical role in immune regulation, and maintains an immune balance. Aims: To appraise the effect of live combined Bifidobacterium, Lactobacillus and Streptococcus Thermophilus tablets on CD4^+ CD25^+ T cells in ulcerative colitis (UC) patients in remission stage. Methods: Thirty-three UC patients in remission stage were divided into UC control group and UC treatment group, patients in the latter group were treated with live combined Biftdobacterium, Lactobacillus and Streptococcus Thermophilus tablets. Healthy subjects were served as normal controls. After the treatment, symptomatic and endoscopic score were assessed. The proportion of CD4^+ CD25^+ T cells and CD4^+ T cells in peripheral blood mononuclear cells (PBMC) and colonic mucosa were determined by flow cytometry, the contents of Foxp3 mRNA in PBMC and colonic mucosa were measured by real-time quantitative reverse transcriptase polymerase chain reaction (RT-PCR). Results: No significant difference was observed in symptomatic and endoscopic score between the UC control group and UC treatment group. Compared with normal control group, CD4^+ CD25^+ T/CD4^+ T ratio increased significantly in PBMC but decreased markedly in colonic mucosa in UC control group (P〈0.05). Compared with UC control group, CD4^+ CD25^+ T/CD4^+ T ratio decreased in PBMC but increased in colonic mucosa in UC treatment group (P〈0.05), and were similar to those of the normal controls. There was no significant difference in the contents of Foxp3 mRNA among the three groups. Conclusions: CD4^+ CD25^+ T/CD4^+ T ratio changes in UC patients in remission stage, which suggests the existence of abnormal immunological regulation. The treatment with live combined Bifidobacterium, LactobaciUus and Streptococcus Thermophilus tablets plays an immunoregulatory role and can normalize the CD4^+ CD25^+T/CD4^+ T ratio.
