感染后肠易激综合征与溃疡性结肠炎缓解期病人直肠黏膜细胞因子的表达(英文) 被引量:2

摘要 目的探讨感染后肠易激综合征(PI-IBS)与溃疡性结肠炎( UC)缓解期病人肠黏膜细胞因子的表达,从神经-免疫机制分析肠易激综合征(IBS)与炎症性肠病(IBD)的相关性。方法 PI-IBS组病人20例,UC组病人45例,正常对照组30例,结肠镜下取直肠黏膜标本,采用免疫组化方法检测其肠黏膜细胞因子的表达情况。结果PI-IBS病人直肠IFN-γ、IL-2阳性表达高于对照组(χ2=14.012、13.931,P<0 .01) ,而与UC病人比较差异无统计学意义(χ2=2.290、2.323,P>0 .05)。PI-IBS病人直肠黏膜P物质(SP)强度均值高于对照组(t=3.722,P<0 .01) ,而与UC病人比较差异无统计学意义(t=1.643,P>0 .05)。直肠黏膜IFN-γ、IL-2阳性表达的PI-IBS病人SP强度均值高于对照组(t=2.301、2.252,P<0 .05) ,与UC病人比较差异无统计学意义(t=1.574,1.676,P>0 .05)。结论 PI-IBS与IBD之间可能存在某种相关性。 Objective To investigate the expressions of cytokines in rectal mucosa in patients with post-infective irritable bowel syndrome (PI-IBS) and in those with ulcerative colitis (UC) in remission and analyze the relationship between irritable bowel syndrome (IBS) and inflammatory bowel disease (IBD) through neuro-immuno-mechanism. Methods Mucosal samples were obtained coloscopically from rectum in 20 patients with PI-IBS, 45 with UC and 30 controls. The expressions of substance P (SP), IL-2 and IFN-γ were detected immunohistochemically. Results The positive rates of IL-2, IFN-γ and SP in rectal mucosa of PIIBS were higher than the controls ((χ^2=14. 012, 13. 931;P〈0.01 for IL-2 and IFN-7; t=3. 722,P〈0.01 for SP), but the differences were not statistically significant as compared with UC ((χ^2 =2. 290,2. 323;P〉0.05 for IL-2 and IFN-γ; t=1. 643,P〉0.05 for SP). The SP in PI-IBS with positive IL-2 and IFN-γ of rectal mucosa was higher than the controls (t = 2. 301,2. 252;P〈 0.05), but no significant difference was noted between this group and UC (t=1. 574,1. 676;P〉0.05). Conclusion Some kind of correlation may exist between the PI-IBS and IBD.
