肝组织中丙型肝炎病毒抗原的表达及其临床意义

Expression of hepatitis C virus nonstructure 4 antigen in liver tissue from patients with hepatitis

目的研究肝炎患者肝组织中丙型肝炎病毒抗原的表达及其临床意义。方法以抗－ＨＣＶＮＳ４ＭｃＡｂ对７７例肝炎患者肝穿标本进行检测。结果ＨＣＶ－Ａｇ（ＮＳ４）呈细小颗粒状位于肝细胞浆中。急慢性肝炎肝组织ＨＣＶ－Ａｇ阳性肝细胞呈散在分布；慢性重型肝炎肝内ＨＣＶ－Ａｇ阳性肝细胞呈弥漫分布。１５例血清抗－ＨＣＶ（＋）和ＨＣＶＲＮＡ（＋）患者中有１２例肝ＨＣＶ－Ａｇ（＋）；１１例血清抗－ＨＣＶ（＋）而ＨＣＶＲＮＡ（－）患者中有３例肝ＨＣＶ－Ａｇ（＋）。结论肝ＨＣＶ－Ａｇ表达与血清ＨＣＶＲＮＡ关系较为密切，ＨＣＶ－Ａｇ表达与ＨＣＶ致病机制有关。

Objective To investigate the expression of HCV NS4 antigen in liver tissue and its clinical significance. Methods We detected 77 liver spicimens from patients with hepatitis by immunohistochemical method with monoclonal antibody to HCV NS4. Results HCV Ag were presented as granule in the cytoplasm of liver cell. The HCV Ag positive hepatocytes were found to distribute sporadically or clustering in the liver tissue from acute and chronic hepatitis patients and diffusely in the liver tissue from chronic fulminant hepatitis patients. HCV Ag positive staining was found in liver tissue from 12 of 15 patients with both HCV Ab and HCV RNA seropositive, whereas 3 of 11 patients with HCV Ab seropositive only. Conclusion The results suggested that the expression of hepatic HCV Ag had distinct relationship with the level of serum HCV RNA, and might take part in the pathogenesis of HCV.
