超声弹性成像与超声造影对乳腺病灶鉴别诊断价值的研究 被引量:32

摘要 目的:探讨超声弹性成像与超声造影技术在乳腺肿瘤的诊断中价值。材料与方法:对行乳腺肿物切除的175位患者共175个病灶术前进行二维超声、彩色多普勒超声、超声弹性成像及超声造影检查。依据超声弹性成像5分法和超声造影相关指标,分别对肿物进行良恶性诊断。结果:175例乳腺病灶,良性123例,恶性52例。超声弹性成像诊断乳腺恶性肿瘤的敏感性为71.2%,特异性为92.7%,准确率为86.3%。超声造影技术诊断乳腺恶性肿瘤的敏感性为78.8%,特异性为96.7%,准确率为91.4%。两者之间的差异无统计学意义。结论:超声弹性成像与超声造影技术有助于乳腺肿物的鉴别诊断,二者具有互补作用。 Objective: To explore ultrasonic elastography and contrast-enhanced ultrasonography in the differential diagnosis of breast tumor. Materials and methods: 175 patients with 175 lesions were enrolled in the study. They were all examined with 2D-ultrasound, color Doppler, ultrasonic elastography and contrast-enhanced ultrasonography before surgery. The diagnosis was made according to ultrasonic elastography and contrast-enhanced ultrasonography respectively. Pathological results were as standard criterion. Results: 123 benign tumors and 52 malignant tumors were confirmed by pathology. The sensitivity, specificity and accuracy for diagnosing malignant breast lesions was 71.2%o, 92.7% and 86.3% by ultrasonic elastography, while 78.8%, 96.7% and 91.4% by contrast-enhanced ultrasonography. Conclusions: Ultrasonic elastography and contrast-enhanced ultrasonography are useful in the differential diagnosis of breast tumor and they can compensate for each other to make an accuracy diagnosis.
