后路椎间盘镜手术治疗腰椎间盘突出合并椎管狭窄症32例疗效观察

Observation of the effect of posterior microendoscropic discectomy in treatment of lumbar disc herniation complicated with lumbar spinal stenosis:report of 32 cases

目的:观察椎间盘镜手术治疗腰椎间盘突出合并椎管狭窄症的临床疗效。方法:椎间盘镜手术治疗32例腰椎间盘突出症合并腰椎管和(或)侧隐窝狭窄,并对术后疗效进行分析。结果:随访时间4～38个月。按Nakai评价标准评定优25例,良4例,可2例,差1例,优良率90.6%(29/32);按脊柱显微内镜治疗腰椎间盘突出症的疗效评价标准评定优25例,良3例,可3例,差1例,优良率87.5%(28/32)。结论:后路椎间盘镜手术治疗腰椎间盘突出合并椎管狭窄症创伤小,术后康复快,疗效佳。

Objective：To observe the clinic effect of microendoscropic discectomy（MED） operation on lumbar disc herniation（LDH） complicated with lumbar spinal stenosis. Methods： Thirty-two cases of lumbar disc herniation complicated with lumbar spinal stenosis or lateral recess stenosis underwent posterior microendoscopic discectomy, and the clinical effects were reviewed and analyzed. Results： The follow-up continued from 4 months to 38 months. The evaluation with Nakai scale revealed excellent in 25 cases, good in 4 cases, fair in 2 cases and poor in 1 case. The rate of good results was 90.6% （29/32）. According to the clinical effect scale, the results by spinal micro-endoscopy therapy were excellent in 25 cases, good in 3 cases, fair in 3 cases and poor in 1 case,with an effective rate of 87.5% （28/32）. Conclusions：Posterior microendoscropic discectomy shows minimal invasion, quick recovery and good clinic effect, it is safe and effective in treatment of lumbar disc herniation complicated with lumbar spinal stenosis.
