本文通过我院临床药师深入临床开展药学监护活动的实践经验,结合理论知识,针对慢性肾脏病患者的用药特点,提出药学监护中需要关注的相关药学信息要点,以期达到合理用药的目的。

According to the the characters of drug applications in people with chronic kidney disease, the ar- ticle combines the theoretical knowledge with the practical experience, which was concluded by the clinical phar- macists in our hospital going deep among the clinical practice and carrying out the pharmaceutical care activities, and also provides the main points of related drug information in pharmaceutical care for rational use of drugs.
