摘要
目的探讨剑桥老年认知量表中文版(CAMCOG-C)测评帕金森病(PD)认知损害的可行性及敏感性。方法运用简易智能状态检查量表(MMSE)、CAMCOG-C等认知量表测评PD认知功能;PD统一评分量表-运动(UPDRS-motor)及Webster评分评定PD的运动功能;PD分期采用Hoehn-Yahr分期。结果89例PD中33例MMSE≤24,与56例MMSE>24的PD比较,两组间年龄、日常生活能力(ADL)及Hoehn/Yahr分期差异有显著性(P<0.001~0.05),而Web-ster评分、UPDRS-motor差异无显著性;89例PD中47例CAMCOG-C<80,与42例CAMCOG-C≥80的PD比较,两组间ADL、Hoehn/Yahr分期、Webster评分及UPDRS-motor差异均有显著性(P<0.01~0.05);与50例正常老年人相比较,56例MMSE>24的PD在CAMCOG-C亚分中语言(语言表达)、记忆(远记忆、近记忆)及执行功能等方面差异有显著性(P<0.001~0.05)。结论CAMCOG-C是测试PD认知功能的有效工具,较MMSE具有更高的特异性和敏感性。
Objective To ascertain the feasibility and sensitivity of Cambridge cognitive examination-Chinese version (CAMCOG-C) as a cognitive assessment tool for patients with Parkinson' s disease(PD). Methods In all of the participants, a battery of neuropsychological tests, including Mini-Mental State Examination (MMSE) and CAMCOG-C, were selected to identify the cognitive deficits. The severity of disease was measured using the Hoe- hn/Yabr. The motor portion of the UPDRS (UPDRS-motor) and Webster scale were used to evaluate motor function. Results Of the 89 PD patients, 33 demonstrated cognitive impairment on MMSE (score≤24) compare with the remaining 56 patients (MMSE 〉 24). There were significant differences in age, activities of daily living (ADL) and Hoehn/Yahr(P 〈 0. 001 ~0.05 ) , not in the Webster scale and UPDRS-motor score. Forty-seven patients of 89 PD were found to have evidence of cognitive impairment on the CAMCOG-C (score 〈 80 ) while the others (score 980) had no dementia. There were significant differences in ADL, Hoehn/Yahr and Webster scale, UP- DRS-motor seore(P 〈0. 01 ~0. 05)between two groups. Compared with the normal elderly, PD with MMSE 〉 24 had significant impairments in the subscores of CAMCOG-C, including memory, praxis (P 〈 0. 001 ~0. 05). Conclusion CAMCOG-C, which tool for PD patients, has more specificity and sensitivity than MMSE. language expression, remote and recent is a viable and useful cognitive screening
