2型糖尿病患者糖基化血红蛋白水平与血脂的关系 被引量:5

Relationship of glycosylated hemoglobin 1c and lipid profile in type 2 diabetes mellitus

摘要 目的探讨2型糖尿病(T2DM)患者糖基化血红蛋白(HbA1c)水平与血脂的相关性。方法检测949例住院的T2DM患者隔夜空腹10h血HbA1c、空腹血糖(FBG)、餐后2-h血糖(PBG)、血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)。结果女性T2DM患者TC、TG和HDL-C均高于男性T2DM患者(P<0.01或P<0.05)。HbA1c与FBG、PBG有显著的相关性(P<0.01)。HbA1c与TC、TG、LDL-C呈显著正相关(P<0.01)。血糖控制差(6.5%<HbA1c≤9%)和控制极差(HbA1c>9%)组的TC、TG和LDL-C显著高于血糖控制好组(HbA1c≤6.5%)。结论T2DM患者血糖水平与脂代谢紊乱密切相关。HbA1c不仅是反映血糖控制的生化指标,而且也可以预示脂代谢的紊乱。 Objective To investigate the relationship of glycosylated hemoglobin 1 c (HbA1c) and the lipid profile in type 2 diabetes meltitus (T2DM). Methods Venous blood samples were collected from 949 hospital T2DM patients after 10 h fasting for the measurements of HbA1c, fasting blood glucose (FBG), postprandial blood glucose FBG), total cholesterol (CH), triglycerides (TG), high-density lipoprotein cholesterol(HDL-C), and low-density lipoprotein cholesterol(LDL-C). Results TC,TG and HDL-C were significantly higher in female T2DM patients than those in male (P〈0.01 or P〈0. 05). There was a highly significant correlation between HbA1c and FBG, PBG(P 〈0. 01). HbA1c was positively correlated with TC, TG, LDL-C. TC, TG and LDL-C were significantly higher in T2DM patients with poor(6.5%《HbA1c ≤9%) or worse (HbA1c〉9%) glycaemic control than those in patients with good glycaemic control (HbA1c ≤6.5%). Conclusion The level of blood glucose is closely correlated with serum lipid profile in T2DM patients. The HbA1c is not only an useful biochemical marker of glycaemic control but also a good predictor of lipid profile disorder.
