靶控输注舒芬太尼对腹腔镜手术患者血流动力学和TXA_2/PGI_2平衡的调节作用 被引量:4

摘要 目的探讨靶控输注舒芬太尼对腹腔镜手术患者血流动力学和血栓素A2(TXA2)/前列环素(PGI2)平衡的调节作用。方法选择60例ASAⅠ~Ⅱ级择期行腹腔镜手术患者随机分为舒芬太尼组和芬太尼组,两组麻醉维持采用靶控输注舒芬太尼或芬太尼。术中和术后连续监测血压和心率(HR)的变化,并于麻醉前(T1)、气腹前(T2)、气腹后15min(T3)和解除气腹后15min(T4)抽取静脉血测定TXA2和PGI2稳定代谢产物TXB2和6-keto-PGF1α浓度。结果芬太尼组气腹后15min平均动脉压(MAP)较麻醉前明显升高,舒芬太尼组MAP无明显变化,舒芬太尼组较芬太尼组明显降低(P<0.05)。两组患者气腹前、气腹后15min和解除气腹后15min的HR与麻醉前比较无统计学差异(P>0.05)。气腹后15min和解除气腹后15min,舒芬太尼组血浆TXB2/6-Keto-PGF1α比值无明显变化,而芬太尼组较麻醉前明显升高(P<0.05),且明显高于同时点的舒芬太尼组(P<0.01)。结论腹腔镜CO2气腹可导致机体血流动力学平衡失调和内源性的TXA2和PGI2水平升高。舒芬太尼靶控输注较芬太尼可更好地维持TXA2/PGI2的平衡,从而更好地维持血管张力和血小板的正常功能,维持血流动力学的稳定,改善微循环灌注,减小腹腔镜手术对机体的损伤。
