目的观察阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者诱导痰中自细胞介素-8(IL-8)和8-异前列腺素F2alpha(8-iso—PGF2α)水平的变化,探讨气道局部炎症和氧化应激与病情严重程度的关系。方法测定30例OSAHS患者和23例健康对照者睡前和晨起诱导痰细胞分类计数和上清液中IL-8和8-iso-PGF2α水平。结果OSAHS组相对于对照组晨起诱导痰中,巨噬细胞百分比减低,中性粒细胞百分比增高,差异均有统计学意义(p〈0.05)。OSAHS组晨起诱导痰IL-8和8-iso-PGF2α浓度分别为(23.6±6.4)ug/L和(149.5±20.0)ng/L,显著高于对照组[(10.0±2.0)ug/L,t=10.946,P〈0.01]和[(91.5±12.4)ng/L,t=12.248,P〈0.01]。OSAHS组睡前诱导痰IL-8和8-iso-PGF2α浓度分别为(10.6±2.0)ug/L和(97.8±10.3)ng/L,与晨起值相比,有显著统计学差异(t=14.038,P〈0.01)和(t=18.191,P〈0.01)。晨起诱导痰IL-8和8-iso-PGF2α浓度均与LspO2呈负相关,与AHI呈正相关。结论OSAHS患者可能存在与IL-8和8iso-PGF2α相关的气道局部炎症反应和氧化应激反应,而且此改变与病情严重程度关系密切。
Objective The aim of the study was to observe the change of interleukin-8 (IL-8) and 8-isoprostaglandin F2alpha (8-iso-PGF2ct) in induced sputum of OSAHS patients, and to explore the relationship of inflammation and oxidative in the airways and the severity of OSAHS. Methods The induced sputum was collected in 30 OSAHS patients and 23 healthy controls In the morning and at pre - sleep. The cell numbers and differentials and the concentration of IL-8 and 8-iso-PGF2α in the sputum was measured. Results OSAHS group showed a significant higher percentage of neutrophils and a lower percentage of macrophages compared to control group ( P 〈0. 05 ). Higher concentrations of IL-8 and 8-iso- PGF2α were found in the morning induced sputum in OSAHS group compared to control group. In OSAHS group, greater concentrations of induced sputum IL - 8 and 8 - iso - PGF2α were found in the morning than that at pre - sleep. A negative correlation was found between IL - 8 and 8 - iso - PGF2α concentrations and LspO2. A positive correlation was found between IL-8 and 8-iso-PGF2 α concentrations and AHI. Conclusions These findings suggest that inflammation and oxidative stress are characteristic in the airways of OSAHS patients,IL-8 and 8-iso-PGF2α may be involved in this process.
