1992～2007年昆明地区大肠癌临床发病特点及发病趋势分析

The Clinical Characteristics and Incidence Tendency of Colorectal Carcinoma in Kunming from 1992 to 2007

目的调查分析研究昆明地区近16a来大肠癌的临床发病特点及发病趋势.方法收集昆明医学院第一附属医院1992年至2007年经病理确诊的1411例大肠癌患者资料.按其发病时间分为1992～1999年和2000～2007年两个连续时间段进行统计分析.结果1992～1999年时间段有555例,2000～2007年时间段856例.前后两时间段比较,收治率上升54.2%.平均年龄由54.00岁上升至58.04岁(P<0.001).青年性大肠癌(≤40岁)比例由19.6%下降至11.3%(P<0.001).直肠癌比例由58.0%下降至50.2%(P<0.05).诊断DukesA期患者比例由5.6%上升到9.7%(P<0.05).结论该地区大肠癌的发病率总体呈上升趋势;直肠癌比例下降;青年性大肠癌比例下降;早期诊断患者比例上升.

Objective To study the clinical characteristics and incidence tendency of colorectal cancer in Kunming. Methods 1 411 colorectal cancer patients pathologically confirmed in the first affiliated hospital of kunming medical college were collected. The clinicopathological data of all patients were divided into two groups according to different phases and reviewed retrospectively. Results Five hundred and fifty-five patients during the period from 1992 to 1999 and eight hundred and fifty-six patients from 2000 to 2007 were admitted. Compared the two phases, the hospitalization number of colorectal cancer increased to 54.2%, the average age increased from 54.00 to 58.04, the proportion of young patients decreased from 19.6% to 11.3% （P 〈 0.05） , the proportion rectal cancer decreased from 58.0% to 50.2% （P 〈 0.05） and the proportion of cases diagnosed in DukesA increased from 5.6% to 9.7% （P 〈 0.05）. Conclusions The incidence of colorectal cancer in Kunming gradually increases. The proportion of rectal cancer decreased. The proportion of young patients decreased. The proportion of cases diagnosed in DukesA increased.
