目的研究p53、p27和mdm2蛋白在胃肠道间质瘤(GIST)中的表达及其与GIST生物学行为的关系,为预测该肿瘤的侵袭潜能提供理论依据和可能的预测指标。方法收集68例手术切除的GIST标本,应用免疫组织化学法检测肿瘤组织中p53、p27和mdm2蛋白的表达,并结合病理形态学及生物学行为评价进行统计学分析。结果68例GIST中p53、p27和mdm2蛋白阳性表达率分别为75%、54.41%和27.94%。p53阳性表达率在极低度侵袭危险性(VLR)、低度侵袭危险性(LR)和高度侵袭危险性(HR)组三组间,细胞核分裂数≤10/50 HPF和>10/50 HPF两组间的差别均有统计学意义(P<0.05),在肿瘤直径≤5 cm和>5 cm两组间差异有高度统计学意义(P<0.01);p27阳性表达率在VLR、LR和HR组间比较有统计学意义(P<0.05),在肿瘤直径≤5 cm和>5 cm两组间差别有高度统计学意义(P<0.01);mdm2阳性表达率在细胞核分裂数≤10/50HPF和>10/50HPF两组间比较差别有统计学意义(P<0.05)。结论p53、p27和mdm2蛋白表达与GIST的发生、发展有关,检测p53、p27及mdm2表达可以作为GIST生物学行为评价的有效指标。

Objective To investigate the expression of p53,p27 and mdm2 protein in gastrointestinal stromal tumor （GIST） , and discuss the relationship between their expression and clincopathological characteristics. Methods Immunohistochemical staining was used to detect the expression of p53 ,p27 and mdm2. in 68 cases of GIST. Their relations with histo-morphology and biological behavior were analyzed statistically. Results In 68 cases GIST, the positivity of p53 ,p27 and mdm2 protein were 75% ,54. 41% and 27.94% respectively. The expression of p53 protein was inversely correlated with the fletcher degree , morphologic variant （ P 〈 0. 05 ） and tumor diameter（ P 〈0. 01 ） of GIST. The expression of p27 protein was inversely correlated with the fletcher degree （ P 〈 0. 05 ） and tumor diameter of GIST （ P 〈 0. 01 ） . The expression of mdnd protein was inversely correlated with the morphologic variant （ P 〈 0. 01 ） of GIST. Conclusion These data shows the expression of p53 ,p27 and mdm2 are related to the ontogenesis and progression of GIST. p53,p27 and mdm2 may be prognostic indicators in GIST.
