彩色多普勒超声在乳腺肿块中的诊断价值

Diagnostic value of color Doppler ultrasound in differentiation between benign and malignant breast lumps

目的:探讨彩色多普勒血流成像(CDFI)对乳腺良恶性肿块鉴别的价值。方法:对50例乳腺肿瘤患者(良性25例、恶性25例)行CDFI检查,首先经二维超声了解其形态、边界、纵横比、内部回声等,然后用CDFI检查,观察肿块内部及周边有无血流,并根据其血流程度分为0级、1级、2级、3级,记录血流分级及RI等血流参数。结果:临床怀疑乳腺肿块50例,超声检出乳腺肿物49例,良性组与恶性组肿物在血流分级上差异存在显著性(P<0.05),频谱分析RI之间差异存在显著性(P<0.001)。结论:综合分析乳腺肿块二维图像及评价CDFI血流分级情况,肿块内血流速度及RI等值有助于对乳腺良恶性肿块的鉴别。

Objective ：To discuss the value of color Doppler flow imaging （CDFI） in differentiating between benign and malignant breast lumps.Methods：Investgating 50 cases of breast tumors patients （25 cases of benign and 25 cases of malignant） with CDFI.First of all, to understand the tumors＇ shape, border, aspect ratio, internal echo and so on with two-dimensional ultrasound, and then to observe the internal and peripheral flow of the lumps with CDFI, ranking into 0, 1,2,3 grade in according to its richness and then to record the blood parameters of flow grade, RI,etc.Results：49 cases of breast tumors were detected by ultrasound in 50 clinical suspected cases.The significant difference （P〈0.05） of flow grade existed between benign and malignant tumors,and the significant difference （P〈0.001） also existed between their RI.Conclusion： Comprehensively analysing the breast lumps＇ two-dimensional image and evaluating the flow grade of CDFI, the blood velocity within the lumps and RI are helpful to differentiate between benign and malignant breast lumps.
