摘要
目的:探讨乳腺导管普通型增生(UDP)、不典型增生(HDP)、癌(Ca)病变发展过程中,Survivin、Cyclin D1和Bcl-2蛋白表达及意义。方法:应用免疫组织化学技术S-P法检测Survivin、Cyclin D1和Bcl-2蛋白在30例UDP、30例HDP、40例癌(Ca)病变中的表达。统计学方法采用卡方检验分析比较各组病变中Survivin、Cyclin D1和Bcl-2蛋白表达的差别及其相关性。结果:①Survivin在UDP、HDP、Ca中的阳性表达率分别为16.67%(5/30)、26.67%(8/30)和95.00%(38/40),Ca与UDP、HDP之间阳性表达率具有显著性差异(P<0.01)。表达范围和表达强度随UDP到HDP到Ca呈逐渐递增的趋势。②Cyclin D1在UDP、HDP、Ca中的阳性表达率分别为23.33%(7/30)、40.00%(12/30)和65.00%(26/40),其中,UDP与Ca之间具有显著性差异(P<0.01),其表达范围和表达强度随UDP到HDP再到Ca也呈逐渐递增的趋势。③Bcl-2在乳腺UDP、HDP、Ca中的阳性表达率分别为83.33%(25/30)、73.33%(22/30)和67.50%(27/40);虽表达率逐渐降低,但组间比较三者之间均无统计学意义(P>0.05)。表达范围和着色强度随UDP到HDP再到Ca呈逐渐递减的趋势。结论:Survivin和Cyclin D1蛋白在乳腺Ca发生的多步骤、多阶段过程中发挥促进作用,Bcl-2蛋白的表达在乳腺组织恶性转化进程中可能起负性调节作用。
Objective: In order to probe the possible role of Cyclin D1, Surivin and Bcl - 2 in carcinogenetic process from usual ductal hyperplasia (UDP) , atypical dutcal hyperplasia (HI)P) to carcinoma in situ and infiltrating carcinoma. Method: The protein expression of the Cyclin D1, Survivin and Bcl- 2 in 30 cases of UDP, 30 cases of HDP and 40 cases of carcinoma in situ and infiltrating carcinoma was detected respectively by immunohis- - tochemistry (S- P). The positive expression rates among these lesions were analyzed by statistics. Result: ①The positive expression rates of survivin in UDP, HDP and breast cancer were respectively 16.67 % (5/30), 26.67 % (8/30), and 95.00 % (38/40), with a significant difference between UDP and cancer (P 〈 0.01) and between HDP and cancer (P〈0.01). Also, the extent and intensity of positive expression were increased from UDP, HDP to cancer . ②The positive rates of Cyclin D1 expression in UDP, HDP and breast cancer were respectively 23.33% (7/30), 40.00% (12/30) and 65.00% (26/40) with a significant difference between UDP and cancer (P 〈 0.01 ) in a similar pattern with Survivin for extent and in- - tensity of expression. ③ The positive rates of Bcl - 2 expression were respectively 83.33 % (25/30), 73.33 96 (22/30) and 67.50(27/40) in these three groups. There was no significant difference among these groups. But, the extent and intensity of positive expression were decreased from UDP, HDP to cancer. Conclusion: Survivin and CyclinD1 play important roles in promoting malignant transforma- - tion, while Blc- 2 may have a negative effect on the malignant transformation.
