四维超声在女性患者大量腹水病因诊断中的应用价值 被引量:4

摘要 目的:研究四维超声在女性患者大量腹水病因诊断中的应用价值。方法:应用Voluson730三维彩超成像系统,应用二维及四维两种超声模式和对经腹及经阴道65例大量腹水的女性患者进行了检查,并将超声检查结果与病理及临床治疗结果进行对照。结果:对大量腹水病因的诊断符合率,四维方法经阴道途径占89%,经腹途径占76%;二维方法经阴道途径占81%,经腹途径占52%;两种途径联合应用,二维与四维超声诊断总符合率分别为81%、89%,两种结果比较差异非常显著,χ2=11.54,P<0.01。结论:四维超声经腹及经阴道两种途径联合应用,对大量腹水病因的诊断结果明显优于二维超声,尤其是对肝硬化、小卵巢癌以及盆腹腔转移结节的诊断符合率、特异度、灵敏度较二维方法均明显提高,成为超声诊断女性患者合并大量腹水病因的重要检查手段。 Objective:To explore the application of 4D ultrasound in the etiological diagnosis of female patients with a large amount of ascites. Methods:65 female patients with a great quantity of ascites underwent both 2D and 4D Voluson 730 color ultrasound via abdomen and vagina simultaneously. The results were compared with pathological and clinical results. Results: The diagnostic accordance rate of a large amount of ascites with 4D ultrasound via vagina and abdomen was 89%, 76%, and that of 2D ultrasound was 81%, 52% respectively. The general uhrasonie diagnostic accuracy of 2D and 4D was 81% and 89 %, respectively, by combination of two methods. There was significant statistical difference between the two groups (X^2 = 11.54, P 〈 0.01 ). Conclusion:The diagnosis accuracy , specificity and sensitivity of 4D ultrasound in patients with a great quantity of ascites is obviously better than that 2D, especially in patients with liver cirrhosis, small ovarian cancer and metastatic nodule of abdominopelvie cavity, which can be used as important auxiliary examination to female patients with a great quantity of ascites.
