羊膜移植在泪小管重建术中的临床应用

Amniotic membrane grafting in reconstruction of canaliculus lacrimalis

目的观察比较显微镜下泪小管断端吻合联合羊膜植入术与常规显微镜下泪小管断端吻合术治疗泪小管断裂的效果。方法60例泪小管断裂吻合术中,32例采用泪小管断端吻合联合羊膜植入术(研究组),28例采用常规显微镜下泪小管断端吻合术(对照组)。结果术后随访观察18个月,置管期内1个月治愈率比较:研究组87.50%(28/32),对照组57.14%(16/28)(P<0.01);拔管后1a治愈率比较:研究组为90.63%(29/32),对照组为69.86%(19/28)(P<0.05),研究组优于对照组。结论泪小管断端吻合联合羊膜植入术能提高泪小管术后的早期修复和最终治愈率,为泪小管手术提供了一个新的治疗途径。

Objective To compare the clinical effect of conventional nasolacrimal anastomosis and nasolacrimal anastomosis with the amniotic membrane grafting in treating breakage of canaliculus lacrimalis under micrscope.Methods In a total of 60 cases with breakage of canaliculus lacrimalis,32 cases treated with amniotic membrane grafting and nasolacrimal anastomosis（study group）,28 cases treated with conventional nasolacrimal anastomosis （control group）.Results Following-up for 18 months,the cure rate was 87.50% （28 in 32） in study group,57.14%（16 in 28） in control group at 1 month with silica gel tube（P〈0.01）;90.63% （29 in 32） in study group,69.86%（19 in 28） in control group at 1 year after extubation of silica gel tube（P〈0.05）,and the clinical effect of study group was better than that of the control group.Conclusions Nasolacrimal anastomosis combined with the amniotic membrane grafting can help the early repair and improve early cure rate.It is a new way to treat the operation of canaliculus lacrimalis.
