摘要
目的研究妊娠早期外周血和蜕膜组织中CD4+CD25+FoxP3+和CD4+CD25+CD127-T细胞的数量变化,并探讨相关性。方法17例原因不明复发性流产(URSA)患者(URSA组)和20名正常健康早孕者(对照组)于人工流产时分别采集外周血和蜕膜组织,Ficoll密度梯度离心法分离单个核细胞悬液,分别加入CD4-PE-Cy5.5、CD25-FITC或CD25-APC和细胞表面标记CD127-PE或细胞内标记FoxP3-PE,流式细胞仪检测样本中CD4+CD25+FoxP3+和CD4+CD25+CD127-T细胞的百分比,并进行线性相关分析。结果与对照组比较,URSA组蜕膜组织中CD4+CD25+FoxP3+和CD4+CD25+CD127-T细胞百分比明显降低(均P<0.05);两组间外周血CD4+CD25+FoxP3+和CD4+CD25+CD127-T细胞百分比比较差异无统计学意义。组内比较显示,两组外周血和蜕膜组织中CD4+CD25+FoxP3+与CD4+CD25+CD127-T细胞百分比比较,差异均无统计学意义(均P(0.05)。相关分析显示,两组CD4+CD25+FoxP3+与CD4+CD25+CD127-T细胞的百分比在外周血(r=0.9858,r=0.6870)和蜕膜组织(r=0.6089,r=0.9583)均呈显著正相关(均P(0.05)。结论URSA的发生可能与蜕膜CD4+CD25+调节性T细胞数量减少有关,而外周血CD4+CD25+调节性T细胞可能不参与URSA的免疫发病机制。细胞表面标记CD127可能成为鉴定和分离CD4+CD25+调节性T细胞的一种新的特异性标记物。
Objective To detect the proportions and role of CD4^+CD25^+FoxP3^+ and CD4^+CD25^+CD127^-T cells in peripheral blood and deciduas during early pregnancy. Methods Peripheral blood and deciduas were obtained during induced abortion from 17 patients with unexplained recurrent spontaneous abortion(URSA) (URSA group) and 20 normal early pregnant women (control group),and mononuclear cells were isolated by Ficoll density gradient centrifugation. The proportions of CD4^+CD25^+FoxP3^+ and CD4^+CD25^+CD127^-T cells of both groups were assessed by flow cytometry,and linear correlation analysis was conducted. Results Compared with control group,both the proportions of CD4^+CD25^+FoxP3^+ and CD4^+CD25^+CD127^-T cells in deciduas of URSA group decreased significantly (P〈0.05). However,there was no significant difference in the proportions of CD4^+CD25^+FoxP3^+ and CD4^+CD25^+CD127^-T cells in peripheral blood between both groups (P〉0.05). There was no significant difference between the proportions of CD4^+CD25^+FoxP3^+ and CD4^+CD25^+CD127^-T cells either in peripheral blood or in deciduas of URSA group (P〉0.05),and the same condition was found in control group. It was indicated by linear correlation analysis that the proportion of CD4^+CD25^+FoxP3^+T cells was positively correlated with that of CD4^+CD25^+CD127^-T cells in URSA group or control group (r=0.9858 and r=0.6870 for peripheral blood,and r=0.6089 and r=0.9583 for deciduas)(P〈0.05). Conclusion The pathogenesis of URSA may be related to the decrease of CD4^+CD25^+ regulatory T cells in deciduas,while CD4^+CD25^+ regulatory T cells in peripheral blood may not take part in the pathogenesis of URSA. CD127 may be a novel cell surface marker for identification and isolation of CD4^+CD25^+ regulatory T cells.
