颈外动脉灌注化疗治疗中晚期头颈部鳞状细胞癌的近期疗效观察

Short-term effect of perfusion chemotherapy via external carotid artery on advanced squamous cell carcinoma of head and neck

目的探讨颈外动脉灌注化疗治疗中晚期头颈部鳞状细胞癌的疗效及影响疗效的因素。方法本组67例患者,全部经病理学检查证实为头颈部鳞状细胞癌患者。采用Seldinger技术,选择性地将导管插入颈外动脉的肿瘤供应支,经导管分次灌注化疗药物。结果67例患者近期临床疗效总有效率(CR+PR)为73.1%,其中多次治疗疗效(79.4%)优于单次治疗(66.7%),多血管型肿瘤的疗效(76.7%)优于少血管型肿瘤(66.7%)。结论中晚期头颈部鳞状细胞癌患者,采用颈外动脉灌注化疗疗效显著,全身副作用小,可以作为中晚期头颈部癌瘤综合治疗的一种重要治疗手段。治疗中要注意肿瘤的血供类型及强调肿瘤的治疗次数。

Objective To explore the effect of the perfusion chemotherapy via external carotid artery on advanced squamous cell carcinoma of head and neck. Methods Sixty - seven patients were pathologically diagoned as advanced squamous cell carcinoma of head and neck. The seldinger technique was adopted to insert a catheter into the external carotid artery on tumor side and perfusion chemotherapy was performed. Results The effective rate was 73.1%. The effect rate of mutiple times perfusion （76.7%） was better than that of one time perfusion （66.7%） ; the effect rate of the tumor rich in blood vessels （76.7%） was better than that of the tumor poor in blood vessels （66.7%）. Conclusion The perfusion chemotherapy via external carotid artery was effective for advanced squamous cell carcinoma of head and neck. The type of blood supply and the therapeutic frequency may affect the effect of persuion chemotherapy.
