弥漫性轴索损伤类型与伤残预后

Classification and Prognosis of Diffuse Axonal Injury of Brain

本文报告56例脑弥漫性轴索损伤（DAI）、约占同期重型颅脑损伤的9．5％．将56例病人根据其临床表现及CT特征不同成分4个类型：皮质下型（Ⅰ型）、中央型（Ⅱ型）、脑干型（Ⅲ型）及混型（Ⅳ型，Ⅳ型又分成Ⅳa及Ⅳb两个亚型）．全组死亡27例，死亡率48．2％．DAI各型中以Ⅰ型预后为好，Ⅲ型及Ⅳb型预后最差，死亡率最高（分别为55．5％及78．6％），而Ⅱ型及Ⅳb型别致残率最高（分别为50％及45.5％）．

56 cases with diffuse axonal injury(DAI) were reported. It was 9.5% of severe braininjuries in same stage. According to patient's clinical manifestation and CT character. We divided thesecases into four types : sub-cortex type (Ⅰ type), center type (Ⅱ type). brainstem type (Ⅲ type) and mixture type (Ⅳ type, Ⅳ type was divided into Ⅳa type and Ⅳb type). Overall mortality was 48.2%.Patients of Ⅰ type had a favourable prognosis. Ⅲ type and Ⅳb type had a high mortality (55.5% and78.6%). Ⅱ type and Ⅳa type had a high maimed rate (50% and 45.5%).
