髌股关节解剖形态的MRI观测及其意义 被引量:9

摘要 目的:采用磁共振对髌股关节的解剖形态进行较全面的定量的观测。方法:选取双下肢功能正常者164例,利用其膝关节MRI图像,分别观测髌骨、股骨滑车及髌腱的解剖形态。结果:(1)髌骨解剖形态:髌骨面角为(125.20±10.11)°,髌骨宽度为(4.17±0.38)cm,髌骨厚度为(2.26±0.26)cm,髌骨外侧面宽度为(2.12±0.25)cm,髌骨面厚度为(1.15±0.20)cm,髌骨外侧面比例为(0.54±0.04),髌骨相对厚度为(0.57±0.06),髌骨长度为(4.30±0.34)cm;其中髌骨宽度、厚度、外侧面宽度、长度、髌骨面厚度在性别间有显著差异(P<0.05)。(2)股骨滑车解剖形态:滑车沟角为(130.71±9.21)°,滑车沟宽度为(3.11±0.31)cm,滑车沟深度为(0.50±0.11)cm,滑车沟外侧面宽度为(1.87±0.23)cm,滑车沟外侧面比例为(0.60±0.05),滑车沟相对深度为(0.16±0.03);其中滑车沟宽度、深度、外侧面宽度在性别间有显著差异(P<0.05)。(3)髌腱长度:髌腱长度为(4.16±0.48)cm,髌骨高度为(0.97±0.16),二者在性别间有显著差异(P<0.05)。结论:髌股关节解剖形态的全面、定量观测结果,将有助于提高髌股关节疾病的疗效及改进髌股关节假体的设计。 Objective: To comprehensively and quantitatively study the patellofemoral anatomic morphology with magnetic resonance imaging (MRI). Methods: 164 healthy adults with normal fimetion of both lower extremities were selected. The patellofemoral anatomic features were measured with their knee MRI. Results: For patella anatomy, patella angle was (125.20± 10.11 )°, patella width (4.17±0.38) cm, patella thickness (2.26±0.26) cm, patella lateral facet width (2.12±0.25) cm, patella facet thickness (1.15±0.20) cm, patella lateral facet ratio (0.54±0.04), patella relative thickness (0.57±0.06), and patella length (4.30±0.34) cm; of them, the width, thickness, lateral facet width, facet thickness and length of patella were of significant statistical difference in gender (P〈0.05). For femoral trochlear anatomy, sulcus angle was (130.71 ±9.21)°, sulcus width (3.11 ±0.31) cm, sulcus depth (0.50±0.11) cm, sulcus lateral facet width (1.87±0.23) cm, sulcus lateral facet ratio (0.60± 0.05), and sulcus relative depth (0.16±0.03); of them, the width, depth and lateral facet width of sulcus were of significant statistical difference in gender (P〈0.05). For patella tendon, patellar tendon length was (4.16±0.48) cm, patellar height (0.97±0.16), both of them were of significant statistical difference in gender (P〈0.05). Conclusions: A comprehensive and quantitative knowledge of the patellofemoral anatomic features is helpful to improve the therapeutic effect of patellofemoral disease and the efficacy of the patellofemoral prosthesis design.
