在生化分析仪上血清铁测定试剂对CRP测定的干扰机理探讨

摘要 目的:通过对血清铁测定试剂对CRP測定的干扰试验,探讨干扰的机理,以制定合理的生化测试顺序、采取适宜的仪器维护保养措施,获得准确的生化检验结果。方法:在日立7060、7180二种机型上,设置不同的测定方式,以及采用纯水代替生化检验试剂进行对照试验。结果:当进行单项血清铁、CRP、Ig测定时,测定结果的重复性良好;当测定顺序为铁→CRP、铁→Ig时,血清铁试剂能干扰测定,使CRP、IgM显著性升高。当将血清铁测定试剂的R2与CRP测定试剂的R2换为纯水时,均不产生干扰。结论:当生化分析仪的试剂针老化、或是清洗能力下降时,血清铁试剂在试剂针内的残留增加而导致干扰反应的发生,使CRP测定结果显著性升高,同时也能干扰大分子IgM的测定。也许可用该干扰试验来评价生化分析仪的性能。 Objective:Through the interference test of serum iron determine reagent interfere with CRP determine,approach the interference mechanism,to institute the reasonable order of determine.Take the suitable maintenance measures for the instrument,to obtain the exact clinical biochemistry results.Methods:Do the interference test on the two instruments type of Hitachi 7060 and Hitachi 7180,setting various determine modes,and use the pure water replace the test reagents.Results:When do the single test for the serum iron,CRP,Ig,the test results are very nice.If the determine order is iron→CRP,iron→Ig,the serum determine reagent can interfere the determination,the result of CRP,IgM increase signification.if the determine reagents R2 of serum iron and CRP are replaced by pure water,have not interference reaction happened.Conclusion:If the reagent probe of the biochemistry analyzer is ageing,or the clean ability decrease,the reagent remain increase in the reagent probe to lead to the interference reaction happened.the CRP determine result can significance increase,also to interfere with the macromolecule IgM determination.Maybe use the interference test to evaluate the biochemistry analyzer performance.
