手术治疗原发性纵隔肿瘤118例报告

Surgical Treatment of Primary Mediastinal Tumors(A report of 118 cases)

本文报告了我院自1965年9月～1997年12月手术治疗纵隔肿瘤118例，完全切除105例，部分切除5例，探查8例，切除率为89％。本组以胸腺瘤最为多见（55例占46．6％），其次为神经源性肿瘤（23例占19．5％）及畸胎类肿瘤（14例占11．9％）。全组无死亡。文中总结了胸腺癌并发重症肌无力的围手术期处理，讨论了巨大纵隔肿瘤手术特殊情况的对策，对半蛤壳状切口在前上纵隔肿瘤切除术中的应用进行了探讨。

This paper reports the results of surgical trcatment of primary mediastinal tumors in 118 cases in our department from Sept 1965 to Dec 1997. In this group, 105 cases were performed radical excision (89%) and there were 55 thymomas (46. 6%), 23 neurogenic tumors (19.5% ),14 teratoblastoma (11.9% ), 10 bronchial cysts (8. 5%) and 16others (13.6). The nature of these tumors was proved by operation and pathology in all patients. In this group,there was no death postoperatively. The periopcrative management of the thymoma with myasthenia gravis,the surgical policy of giant mediastinal tumors and the application of hemiclamshcll incision in the anterior-supcrior mediastinal tumors are discussed in the paper
