乌司他丁治疗急性胰腺炎相关性肺损伤疗效分析

摘要 目的探讨乌司他丁在治疗重症急性胰腺炎(severe acute pancreatitis,SAP)时对急性胰腺炎相关性肺损伤(acute pancreatitis associated lung injury,APALI)的治疗作用。方法将我院2002年1月至2007年12月间44例SAP合并APALI患者进行分组,治疗组23例,行常规+乌司他丁治疗,对照组21例,行常规治疗,对照分析治疗期间呼吸频率、血气分析结果、胸片变化及疾病进展和预后情况。结果治疗各阶段乌司他丁治疗组较对照组肺损伤程度明显见减轻(P<0.05),且发生急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)和多脏器功能障碍综合征(mutiple organ dysfuntion syndrome,MODS)的发生率要低于对照组,死亡率明显降低(P<0.05)。结论乌司他丁对SAP合并APALI有显著的治疗作用,可以抑制炎症反应,降低死亡率。 Objective To study the remedial effect of Ulinastatin(UST) on the severe acute pancreatitis(SAP) complicating acute pancreatitis associated lung injury(APALI) .Methods Randomize the 44 cases who were suffering from SAP complicating APALI in our hospital into 2 groups: twenty--one patients in control group were treated with conventional managements.another twenty-- three patients in therapeutic group were treated with conventional managements and UST;Contrast analyze the breathing frequencn, result of blood gas analytic, X-roy of lung change, the progression of disease and prognosis status.Results In all phases, the degrees of lung injury in the therapeutic group was lessened singificantly than the control group (P〈0.05);the incidence of ARDS and MODS in the therapeutic group was lower than the control group;and the death rate of the therapeutic group degraded obviously (P〈0.05).Conclusion UST have the significant therapeutical effect to the SAP complicating APALl,and it can inhibit the inflammatory reaction and cut down the death rate.
