胆囊息肉样病变超声诊断的临床分析

The Ultrasound Diagnosis and Analysis of the Polyoid Lesion Gallbladder

目的探讨胆囊息肉样病变的声像图表现与病理改变的关系,分析超声诊断对手术指征的临床价值。方法对116例经手术和病理切片证实的胆囊息肉样病变的声像图和病理诊断进行回顾性分析。结果胆固醇息肉86例(74.1%),73例表现为多发病灶,平均直径6.0mm,蒂窄;乳头状腺瘤11例(9.5%),均表现为单发病灶平均直径12.3mm;胆囊炎性息肉13例(11.2%);腺肌瘤4例(3.4%),腺癌2例(1.7%)。结论根据超声声像图上息肉数目、大小、与胆囊壁的关系等情况可初步判断其性质,指导临床选择治疗方案。

Objective To investigate the relation between the Ultrasonographic images and the pathological aspect of polypoid lesions of gallbladder（PLG） and the usage in directing clinical management.Methods Retrospectiv analysis was performed for the relation between the ultrasonographic images and pathological aspect of 116 cases with polypoid lesions of gallbladder who were certified by the operation and pathologies.Results Among them,there are 86 cases with cholesterol polyps.Most of cholesterol polyps are multifocal, narrow pedicle and 6.0mm in average diameter.11 cases of them are Papillary adenoma patients,occur aloneand 12.3mm in average diameter.And there are 13 cases with Gallbladder nflammatory polyps,4 cases with adenomyoma and two cases with adenocarcinoma. Conclusion The nature of polypoid lesion is determined by numbers,size of pedicle of the polyp with ultrasonography to help clinician out selecting treatments.
