急性脑梗死患者血管介入治疗后纤溶活性变化的初步观察 被引量：6

摘要 目的初步探索脑血管球囊成形支架置入术(PTAS)联合抗凝及抗血小板治疗对急性动脉粥样硬化性脑梗死患者体内纤溶系统的影响。方法急性动脉粥样硬化性脑梗死患者96例为观察组,非脑血管病患者98例为对照组。观察组按治疗方法不同又分为阿司匹林组(ASP组)、低分子肝素组(LMH组)和PTAS组。以双抗夹心酶联免疫吸附法检测患者入院后第1天和第7天血浆血管性假血友病因子(vWF)、组织型纤溶酶原激活物(tPA)及其抑制物-1(PAI-1)水平。结果观察组第1天和第7天血浆tPA、PAI-1和vWF抗原水平均显著高于对照组;PTAS组第7天vWF显著高于ASP组和LMH组,三指标水平均较第1天升高,且tPA,vWF的升高幅度显著大于、PAI-1水平增加则显著小于ASP组和LMH组。结论PTAS联合抗凝及抗血小板治疗在维持急性动脉粥样硬化性脑梗死患者体内纤溶活性方面不差于甚至可能优于单纯抗凝或抗血小板治疗。
