摘要
目的探讨神经传导速度(NCV)和H反射对糖尿病周围神经病(DPN)的诊断价值。方法糖尿病周围神经病患者(DM组)和健康对照者(对照组)各60例,用肌电图诱发电位仪检测正中、尺、腓、胫神经NCV,并在腘窝引出H反射。结果DM组的NCV及H反射与对照组比较异常率明显增高(P<0.01);下肢运动神经传导速度(MCV)异常率高于上肢,感觉神经传导速度(SCV)异常率高于MCV。结论NCV及H反射检查可作为DPN的检测手段之一。
Objective To explore the diagnostic value of nerve conduction velocity (NCV) and H-reflex in patients with diabetic peripheral neuropathy. Methods Fifty patients with diabetic peripheral neuropathy served as experimental group and 60 healthy subjects as the control group. NCV of the madian, ulnar, peronea Ⅰ and tibial nerves were measured and H-reflex was elicited by electromyography. Results A significant difference in NCV and H-reflex was noted between experimental group and control group. H-reflex and SCV were more sensitive than MCV. The abnormal rate of NCV and H-reflex in experimental group was 46% -72%. The abnormal rate of lower limb was higher than that of upper limb, and the rate of SCV was higher than that of MCV. Conclusion Electromyography might be one of reliable and sensitive diagnostic measures for early diabetic peripheral neuropathy.
