残胃癌外科手术治疗经验及消化道重建新方法

Surgical Treatment of Cancer of Gastric Stump and Reconstruction of Alimentary Tract

１９７２年１月～１９９６年１２月，收治残胃癌３９例，其发病率为１．８５％（３９／２１１０）。本文对其发生病因、发病率、外科处理及防治措施进行探讨。强调外科治疗要早诊早治。残胃癌要残胃全切除合并邻近脏器联合切除及Ｄ＋２、Ｄ３淋巴结廓清术。消化道重建方式推荐空肠原位间置代胃术、空肠Ｒｏｕｘ－Ｙ重建消化道新方法。才能取得满意疗效。

Thirty-nine cases of stomach stump cancer with an incidence rate of 1.85% (39/2110) were treated surgically during the period from January 1972 to December 1996. It is emphasized that early diagnosis and treatment are very essential.Radical resection of the entire gastric stump and the adjacent organs involved and elimination of lymph nodes (D2+ or D3) are mandatory. A Roux Y esophago jejunostomy for reinstitution of alimentary reconstruction is justified.
