uPA、β-catenin表达及MVD与甲状腺癌侵袭转移的关系

uPA, β-catenin expression status and microvessel density correlated to invasion and metastasis in thyroid carcinoma

摘要 目的研究甲状腺癌组织中尿激酶型纤溶酶原激活物(uPA)、β-连环素蛋白(β-catenin)表达及其与血管生成的关系,探讨其在甲状腺癌侵袭、转移中的作用。方法采用免疫组织化学SP法检测90例人甲状腺癌组织中uPA、β-catenin及CD_105的表达,并对CD105表达阳性血管进行微血管密度(MVD)计数,结合甲状腺癌的临床病理特征进行分析。结果甲状腺癌组织中uPA阳性表达、β-catenin异常表达及MVD值与甲状腺癌组织分型有关,在淋巴结有无转移间以及病理分期Ⅰ-Ⅱ期、Ⅲ-Ⅳ期之间差异有统计学意义(P〈0.05)。甲状腺癌组织的MVD值与uPA阳性表达、β-catenin异常表达呈正相关(r=0.766,P〈0.05);uPA阳性表达与β-catenin异常表达呈正相关(r=0.922,P〈0.05)。结论甲状腺癌uPA阳性表达率高,广泛存在β-eatenin膜表达率降低及胞质和(或)胞核内异常表达,且与其组织类型、临床分期及侵袭转移密切相关。MVD计数与甲状腺癌组织类型、临床分期及侵袭转移密切相关。uPA、β-catenin在甲状腺癌肿瘤血管生成中具有协同作用。uPA、β-catenin及MVD可作为甲状腺癌患者预后判断的指标。 Objective To investigate the expression of urokinase-type plasminogen activator (uPA) and β-catenin proteins and their relationship with tumor angiogenesis, and explore their correlation with invasion and metastasis of thyroid carcinoma. Methods SP immuno-histoehemistry was performed to detect the expression of uPA, β-eatenin and CD105 in 90 cases of thyroid carcinoma tissues. MVD counting was perfortned and analyzed in conjunction with the clinicopathological features of human thyroid carcinomas. Results The positive rate of uPA expression and abnormal expression rate of β-eatenin and the MVD value was positively correlated with histological typing of thyroid carcinoma. There was significant difference in thyroid carcinoma with lymph node metastasis, which was significantly higher than those cases without lymph node metastasis ( P 〈 0.05 ) , as well as between clinical stage Ⅲ L Ⅳ and Ⅰ - Ⅱ. The MVD value was significantly correlated with the positive expression of uPA and abnormal expression of β-eatenin in thyroid. There was statistically significant correlation between the expression of uPA and β-eatenin. Conclusion The expression of uPA protein in thyroid carcinoma was higher than those in normal. A reduced membranous expression rate and abnormal nuclear/cytoplasmic expression of β-eatenin was observed in thyroid carcinoma cells, which was positively correlated with histological typing and clinical staging, invasion and metastasis in thyroid carcinoma, uPA and β-catenin were cooperated in angiogenesis of thyroid carcinoma, MVD was positively correlated with invasion and metastasis in thyroid carcinoma. They were hopeful to be an important prognostic indicator of thyroid carcinoma.
