2组含顺铂的化疗方案同步放化疗对晚期非小细胞肺癌的疗效观察 被引量:1

摘要 目的选取泰素加顺铂(TP组)和健择加顺铂(GP组)两种方案联合同期放疗,对晚期非小细胞肺癌疗效和毒副反应进行比较。方法63例晚期非小细胞肺癌(ⅢA,ⅢB或Ⅳ)分为2组,TP组32例,GP组31例,比较2组的疗效及毒副反应。TP组Tax用量为135mg/m^2,第1天静注;DDP用量为80mg/m^2,第1天静注,第4周重复;GP组Gem用量为1000mg/m2,第1天和第8天静注;DDP用量为80mg/m^2,第1天静注,第4周重复。放射治疗采用常规放疗技术,常规分割(2Gy/次,5d/周),总剂量40-66 Gy/4~6周。结果总有效率TP组为65.6%,GP组为64.5%(P〉0.05);2组的1、2、3年生存率分别为55.11%、26.13%和12.13%(TP组),59.6%、28.23%和13.41%(GP组),两种方案的主要不良反应均为骨髓抑制,白细胞下降至Ⅲ、Ⅳ度者,I'P组为56.25%,GP组为54.89%(P〉0.05),2组的放射性食管炎TP组为18.75%,GP组为16.13%(P〉0.05);放射性肺炎TP组为3.13%,GP组为12.90%(P〈0.05)。结论TP方案和GP方案同期放化疗治疗晚期非小细胞肺癌效果确实,且较一致,耐受性均较好,但使用GP方案要重视放射性肺炎的发生。
