血管转换酶抑制剂治疗充血性心力衰竭65例疗效分析

Clinical study on angiotensin-converting enzyme inhibitor to cure congestive heart failure

目的研究血管转换酶抑制剂(ACEI)治疗心力衰竭(CHF)的临床效果。方法将96例充血性心力衰竭患者随机分成治疗组和对照组,治疗组(n=65)在常规治疗基础上加用血管转换酶抑制剂,从小剂量开始逐步达到"靶剂量",对照组(n=31)仅给予常规治疗(强心、利尿、扩血管)。两组服药前后检查心率、6min步行测验、血压、LVDD、LVEF、心功能,并进行比较。随访3年。结果ACEI治疗CHF后心率减慢,血压下降,6min步行距离增加,治疗组临床指标改善明显,心功能改善总有效率达89.2%,而对照组仅67.7%,两组比较具有统计学意义(P<0.01)。结论ACEI治疗CHF可改善心功能,提高生存率和生活质量,防止和延缓心肌重塑的发展,从而降低心力衰竭的死亡率,值得推广。

Objective To study the clinical effect of angiotensin - converting enzyme inhibitor （ACEI） to treat congestive heart failure （CHF）. Methods Chose 96 cases the congestive heart failure were randomly divided into treatment and control groups. The treatment groups （ n = 65 ） used ACEI on routing treatment base. Dose from low to ＇ target dose＇ step to step. In the control groups （ n = 31 ） were given only routine treatment （ Cardiotoni, Diuretic and Vasodioctive）. Test heart rate, Blood pressure, the distance for 6 minutes walking test, LVDD, LVEF and heart function grade. Then compared the changes before and after treatment and assessed the clinical effects. Patients were followed up for 3 years. Results After ACEI treated CHF, the heart rate decreased, blood pressure lower, 6 - minute walking distance increase. The heart function improved ob^iously, the valid rate reached 89.2% , the control groups only 67.7%, the statisticalogy signitieantly compared on two groups（ p 〈0.01 ）. Conclusion ACEI treat CHF improves the heart function, survival rate and living quality. The cardie remodeling developing were prevent and postpone. So dead rate and admission reduced.
