十二指肠损伤15例诊治体会

目的探讨十二指肠损伤的诊断和外科治疗方法。方法回顾性分析15例十二指肠损伤患者的临床资料。结果15例患者仅3例术前明确诊断,术中确诊11例,术中漏诊1例,误诊率7%,治愈13例,治愈率87%,死亡2例,死亡率13%。十二指肠损伤的早期诊断和早期及时手术是非常重要的。结论十二指肠损伤的病情复杂,目前尚无好的特异性检查,术前明确诊断极为困难,术中亦容易漏诊,处理困难。结论十二指肠修补术、十二指肠空肠Roux-en-Y吻合术和Cogbill改良憩室化手术再简化手术是治疗十二指肠损伤的实用方法。

Objective To study the early diagnosis and suitable surgical treatment for duodenal injury. Method：Clinical data of 15 patients with duodenal injury were analyzed retrospectively.Result： 15 cases underwent surgical treatment. 13 were cured. 2 died. Early diagnosis and treatment for duodenal injury were very important. Conclusion：Primary repair and Roux-en-Y duodenojejunostomy and Modificed CogbiU＇s technigues for diverficalization are practical operation.
