食管癌手术后早期中医药治疗临床研究

Study on treatment of early postoperative period of esophageal cancer with TCM

目的评价食管癌手术后胃肠功能紊乱早期应用中药治疗方案的临床疗效。方法采用前瞻性设计、多中心、随机、对照方法选择食管癌根治手术(术中切断迷走神经)后早期胃肠功能紊乱的患者100例,分为治疗组和对照组各50例,治疗组早期应用中医药治疗方案治疗。观测2组患者对临床症状、胃窦蠕动、胃排空时间、胃功能恢复的影响。结果中医证候疗效:治疗组临床痊愈28例,显效20例,显效率98%;对照组临床痊愈9例,显效18例,显效率54%。经统计学处理2组疗效有显著性差异。B超检查:手术后2组的胃窦蠕动出现时间、胃部分排空时间、胃功能恢复到手术前水平的时间经统计学处理有显著性差异;2组患者服药后未见明显不良反应和毒副作用;2组对血常规、小便常规、大便常规、转氨酶、肾功能、心电图等安全性指标无明显影响。结论食管癌根治术后早期应用中医药治疗,对术后胃肠功能紊乱有较好的近期临床疗效,且安全性好。

Objective It is to observe the clinical curative effect of traditional Chinese medicine （TCM） on postoperative gastrointestinal dysfunction of esophageal cancer. Methods 100 cases with gastrointestinal dysfunction after radical operation- cutting off vagus nerve on esophageal carcinoma were chose by the method of perspective designing and stochastic contrast. There were both 50 cases in treatment group and control group. The treatment group was treated with TCM protocols in early period. The influence of clinical symptoms, gastric antrum peristalsis, gastric emptying time and gastric function were observed. Results The curative effect of TCM Syndromes： 28 cases were cured, 20 cases were marked effect and the remarkable curative effect were 98 % in treatment group. 9 cases were cured, 18 cases were marked effect and the remarkable cura- tive effect were 54 % in control group. There were significant differences between the two groups. 13- ultrasound showed： After operation, the time of gastric antrum peristalsis starting, gastric emptying and gastric function recovering as well as pre-operation showed significant differences between the two groups. Patients in the two groups had no marked expression of adverse reaction or side effect. There was no influence of blo^d routine, stool routine, urine routine, renal function, ECG examination and so on. Conclusion TCM treatment for early postoperative period is effective and safe for curing gastrointestinal dysfunction after radical resection of esophageal cancer.
