Status of iodine deficiency disease and implement condition of intervenient measure in 5 higher risk area in Inner Mongolia in 2007

摘要 目的了解我区碘缺乏病高危地区是否存在新发碘缺乏病例和地方性甲状腺肿流行现况及综合防治措施落实情况。方法新发碘缺乏病人诊断采用地方性克汀病和地方性亚临床克汀病诊断标准(WS104-1999)诊断;智商测查采用中国瑞文农村修订版进行测验;儿童甲状腺检查采用B超和触诊两种方法;盐碘测定采用半定量试剂进行;尿碘测定采用砷铈催化分光光度测定方法(WS/T107-2006)。结果8~10岁儿童甲肿率触诊检查为5.6%,B超检测为4.6%;8~10岁儿童尿碘中位数为239.8μg/L,66.95%尿碘值分布在100-300μg/L之间;8~10岁儿童平均IQ值为104.04±13.47;家庭主妇尿碘中位数为222.45μg/L;调查的1389户居民中有97.3%的在食用碘盐,仅有2.7的居民在食用无碘盐。结论5个调查的旗县区碘缺乏病病情得到有效控制,人群碘营养水平适宜,儿童的智力得到有效保护,食盐加碘为主消除碘缺乏病防治措施得到有效的落实,目前尚未发现新发克汀病病例。 Objective To understand higher risk area of iodine deficiency disease whether incidents endemic cretinism, prevalent status of endemic goiter and implement condition of preventive measure. Methods Patients of endemic cretinism were diagnosed by "standard of diagnosis of endemic cretinism and endemic subclinical cretinism (WS104 - 1999) " . IQ was tested by chinese auspicious article countryside revised edition. The thyroid size was examined by palpation and B ultrasound, iodine concentration in salt was detected by half quantitative reagent and that in urine by the method of As3 + - Ce4 + catalytic spectrophotometry ( WS/T107 - 2006) . Results The rates of goiter were 5. 6% by palpation and 4. 6% by B ultrasound, the median of urinary iodine was 239. 8μg/L, the range of urinary iodine was 100 -300μg/L for 66. 95%, and the average of IQ was 104. 04 ± 13.47 for the pupils aged 8 - 12 years. The median of urinary iodine was 222. 45μg/L for the housewives. 1389 families were investigated, the percent of used iodine salt was 97.3%, in which the percent of used non - iodine salt was 2. 7%. Conclusion Prevalence of iodine deficiency disease in 5 counties is controlled effectively. The current situation of iodine nutrition is suitable. Child's intelligence obtains the effective protection. The edible salt adds iodine primarily to eliminate IDD is effective. None cases of endemic cretinism continued to emerge at present.
