浆母细胞性淋巴瘤的临床病理观察

The Clinical and Pathological Observation of Plasmablastic Lymphoma

目的观察浆母细胞性淋巴瘤的临床及病理特征,以积累经验,指导临床工作。方法收集我院诊治的1例浆母细胞性淋巴瘤患者,结合病理学特点、免疫组化、临床特征及相关文献,对其进行临床资料及文献的回顾分析。结果肿瘤细胞表现为中等大小的核,分裂像多,具有浆细胞样的形态特征;免疫组化:LCA(+),Vs38c(+),浆细胞(+),CD38(+),CD56(+),CD79a(+),IgG(+),CD3(-),CD10(-),CD20(-),CD30(-),CD45RO(-),Ki67阳性率75%;骨髓像涂片中可见少数浆细胞样的肿瘤细胞。结论浆母细胞性淋巴瘤预后差,免疫组化对诊断至关重要,临床应注意鉴别诊断。

Objective To investigate the clinical and pathological features of plasmablastic lymphoma, for accumulating clinical experience and improving their treatment.Methods The case of plasmablastic lymphoma was studied by pathological, immunohistochemical, clinical characteristic and literature.Results Tumor tissue was composed of plasmablast--like cell, which was observed many pathological mitotic and moderate large cells. The tumors cells were positive for LCA, Vs38c, Plasma Cell, CD38, CD56, CD79a and IgG. The tumors cells were negative for CD3, CD10, CD20, CD30, CD45RO and Ki67（75%）. A few plasmablast--like cell were found in bone marrow smear.Conclusion Plasmablastic lymphoma is a poor prognostic neoplasm. Immunohistoehemistry play important roles to the diagnosis and differential diagnosis
