摘要
目的:探讨L-selectin、CD44、α6β1对宫颈癌淋巴转移的作用方法:应用免疫组织化学方法检测35例宫颈鳞状细胞癌原发灶及其淋巴转移灶中L-selectin、CD44、α6β1的表达。结果:L-selectin淋巴转移灶中阳性率明显低于原发灶(P<0.05);CD44淋巴转移灶中阳性率与原发灶无明显区别(P=1.230 5);α6β1淋巴转移灶中阳性率明显高于原发灶(P<0.01);L-selectin与CD44、L-selectin与α6β1在原发灶中表达水平之间存在明显相关(r=0.873 7,P<0.01和r=0.795 7,P<0.01),CD44、α6β1在原发灶中表达水平呈明显负相关(r=-0.658 3,P<0.01);淋巴转移灶中L-selectin、CD44、α6β1表达水平之间均不存在相关;L-selectin原发灶与淋巴转移中表达水平之间存在明显相关(r=0.753 4,P<0.01),CD44原发灶与淋巴转移中表达水平之间无明显相关,α6β1原发灶与淋巴转移中表达水平之间存在明显负相关(r=-0.536 1,P<0.01)。原发灶中L-selectin表达与肿瘤分化、淋巴结转移存在相关(r=0.842 0,P<0.01和r=0.768 9,P<0.01);CD44表达与肿瘤侵犯深度、淋巴结转移之间存在相关(r=0.678 2,P<0.01和r=0.863 4,P<0.01);α6β1表达与淋巴结转移存在相关(r=0.654 8,P<0.01)。结论:L-selectin、CD44、α6β1都与宫颈癌淋巴转移相关,其中L-selectin、CD44在淋巴转移的始动阶段发挥重要作用。
Objective: To explore the effect of L - selectin, CD44andα6β1 on lymphatic metastasis in cervical carcinoma. Methods: The expression of L - selectin, CD44 and α6β1 in original carcinoma and lymphatic metastasis of 35 cases were detected by immmunohistochemistry method. Results: The positive rate of L - selectin in original carcinoma was higher than that in lymphatic metastasis (P 〈 0. 05) ; there was no significant difference in the positive rate of CD44 between original carcinoma and lymphatic metastasis (P = 1. 230 5) ; the positive rate α6β1 in lymphatic metastasis was higher than than that in original carcinoma (P 〈0. 01 ) ; there was positive correlation between positive rates of L - selectin, CD44 andα6β1 in original carcinoma (r --0.873 7, P 〈0.01; r =0.795 7, P 〈 0.01 ), there was negative correlation between positive rates of CD44 and α6β1 in original carcinoma (r = - 0. 658 3, P 〈 0.01 ) ; there was no correlation between the expression of L - seleetin, CD44 and α6β1 in lymphatic metastasis ; there was positive correlation in the expression of L - selectin between original carcinoma and lymphatic metastasis ( r = 0. 753 4, P 〈 0. 01 ) , there was no correlation in the expression of CD44 between original carcinoma and lymphatic metastasis, there was negative correlation in the expression of α6β1 between original carcinoma and lymphatic metastasis ( r = - 0. 536 1, P 〈 0. 01 ) ; there was positive correlation between the expression of L - selectin and tumor differentiation, lymphatic metastasis ( r = 0. 842 0, P 〈 0. 01 ; r = 0. 768 9, P 〈 0. 01 ) ; there was positive correlation between the expression of CD44 and invasive depth, lymphatic metastasis ( r = 0. 678 2, P 〈0. 01; r =0. 863 4, P 〈0. 01); there was positive correlation between the expression ofα6β1 and lymphatic metastasis (r =0. 654 8, P 〈 0. 01 ) . Conclusion: The expression of L - seleetin, CD44 and α6β1 are related to lymphatic metastasis of cervical carcinoma, and L- selectin and CD44 play an important role in early lymphatic metastasis.
