老年高血压病患者活性维生素D与尿微量白蛋白及慢性肾脏疾病的关系 被引量:2

摘要 目的探讨老年高血压病患者活性维生素D与尿微量白蛋白(MAU)及慢性肾脏疾病(CKD)的关系。方法采用化学发光法检测196例CKD1~4期非透析老年高血压病患者的血清25(OH)D水平,分别检测尿MAU、血清肌酐(Cr)、总胆固醇(TC)、甘油三酯(TG)、空腹血糖(FBG)、钙(Ca)、磷(P)、甲状旁腺素(PTH)等指标。结果 MAU组的尿MAU、血清Cr、TC、TG及FBG水平高于非MAU组,血清25(OH)D水平低于非MAU组(P<0.05),CKD3期组的血清25(OH)D水平低于CKD1~2期组,尿MAU水平高于CKD1~2期组,CKD4期组的血清25(OH)D水平低于CKD3期组,尿MAU及血清PTH水平高于CKD3期组(P<0.05),血清25(OH)D与估算肾小球滤过率(eGFR)呈正相关,与尿MAU呈负相关(P<0.05)。结论 CKD1~4期非透析患者存在活性维生素D不足或缺乏,并与eGFR及尿MAU密切相关,应重视活性维生素D在CKD治疗中的重要作用。
