整合素α_5β_1、Fn及CD_(44v6)在卵巢上皮性肿瘤中的表达及其意义

Clinical significance of integrinα_5β_1,Fn and CD_(44v6) expression in ovarian epithelial neoplasms

目的探讨整合素α_5β_1(integrinα_5β_1)、纤维连接蛋白(Fn)及CD_(44v6)与卵巢癌淋巴结转移的关系。方法用免疫组织化学Elivision法检测80例卵巢上皮性肿瘤中integrinα_5β_1、Fn及CD_(44v6)的表达;运用图像分析系统对各指标进行定量检测。结果定性及半定量结果:integrinα_5β_1、Fn的表达按良性、交界性及恶性的顺序呈递减趋势,组间差异均有统计学意义(P<0.05);CD_(44v6)的表达则呈递增趋势,组间差异有统计学意义(P<0.05);integrinα_5β_1、Fn淋巴结转移组与未转移组间表达差异有统计学意义(P<0.05);integrinα_5β_1与Fn的表达呈正相关,与CD_(44v6)的表达呈负相关。定量检测结果:inte- grinα_5β_1表达的阳性细胞面积按良性、交界性、恶性的顺序呈递减趋势,灰度呈递增趋势;CD_(44v6)的表达则相反,面积呈递增趋势,灰度呈递减趋势。结论integrinα_5β_1、Fn表达下降或缺失,CD_(44v6)表达升高,提示肿瘤进展、恶性度高、预后不良;计算机图像分析系统对卵巢上皮性肿瘤的自动化诊断、淋巴结转移及预后评估有重要意义。

Objective Through the investigating the molecular expressions of integrinα_5β_1,Fn and CD_(44v6) in ovarian epithelial neoplasms,this study is trying to explore the relationship between the lymphatic spread of the tumor with these molecules.Methods The expression of 80 cases of integrinα_5β_1,Fn and CD_(44v6) was examined through ElivisionTM immunohistochemistry method in ovarian epithelial neoplasm.The system of image analysis was used to measure the expression of various molecules quantitatively.Results Qualitative and semi-quantitative results:The expression levels of integrinα_5β_1 and Fn in the ovarian epithe- lial neoplasms had downward tendency in the order of benign,boundary and malignant neoplasms and there was a significant difference in the expression levels of integrinα_5β_1 and Fn(P<0.05).On the other hand,the expression of CD_(44v6) had upward tendency in that order and there was a significant difference in the expres- sion levels of CD_(44v6)(P<0.05).There was a significant difference in the expression levels of integrinα_5β_1 and Fn between the group with metastases and the group without metastases(P<0.05).Meanwhile,there was posi- tive correlation between the expressions of integrinα_5β_1 and Fn,and a negative correlation between the ex- pression of integrinα_5β_1 and CD_(44v6).Quantitative examination results:The total positive cell area of the integrinα_5β_1 expression in the tumor had downward tendency in the order of benign,boundary and malignant cases, and the gray degree had upward tendency.On the contrary,the total area of CD_(44v6) expression had upward ten- dency in that order,and its gray degree had downward tendency.Conclusion Decreasing or lack of the ex- pression of integrinα_5β_1 and Fn as well as uprising of the expression of CD_(44v6) suggest the rapid development, high degree of malignancy and poor prognosis of tumour;The study also shows the improtance of using the computer image analysis for automatic diagnosis of the ovarian epithelial neoplasms,lymphatic metastases and its prognostic estimation.
