环磷酰胺联合G-CSF动员慢性粒细胞白血病患者外周血造血干细胞

Cyclophosphamide combined with G-CSF for mobilizing peripheral blood stem cells in patients with chronic myelogenous leukemia

目的评价环磷酰胺(Cy)联合粒细胞集落刺激因子(G-CSF)动员慢性粒细胞白血病(CML)患者外周血造血干细胞的动员、采集效果及不良反应。方法选择符合诊断的CML患者7例,无心、肺、肝、肾等重要脏器功能损害。动员方案为Cy+人基因重组粒细胞集落刺激因子(rhG-CSF),于化疗后WBC降至1.0×10~9/L时开始用rhG-CSF,用CS3000 plus血细胞分离机采集干细胞,-80℃低温冰箱冻存,并作单个核细胞(MNC)计数及CD_(34)^+细胞测定。结果Cy平均剂量为4.0(3.0～5.0)g,rhG-CSF的剂量均为300μg/d,平均5.8(3～9)d;其中仅1例患者动员2次,余6例均动员1次;共采集2次,每次循环血量为10～13 L;采集的MNC达(2.94～5.45)×10~8/kg,CD_(34)^+细胞数达(2.15～6.59)×10~6/kg;患者能耐受整个动员和采集过程,无一例因化疗的不良反应而中止动员;移植后全部患者均迅速恢复了造血功能。结论Cy联合G-CSF的动员方案对CML患者是安全、有效的。

Objective To investigate the clinical value of cyclophosphamide combined (Cy) with G- CSF for mobilizing peripheral blood stem cells in patients with chronic myelogenous leukemia (CML).Meth- ods Seven CML patients treated at ours hospital,without organ damage such as heart,lung,liver,rend and else,were chosen into our research.The mobilization regimen was Cy plus rhG-CSF.Harvesting of peripheral blood stem cells with CS-3000 Plus cell separator.Results The average dosage of Cy was 4.0(3.0～5.0)g.The doses of G-CSF was 300μg/d,The average days of G-CSF was 5.8(3～9)d.All the 7 patients were collected twice.All of the patients could tolerate the treatment regimens and obtained the hematogenic restoration. Conclusion Cy combined with G-CSF is safe and effective for mobilizing peripheral blood stem cells in pa- tients with CML.
