摘要
目的:监测大肠埃希氏菌临床分离株的耐药性,更好的指导临床合理用药。方法:常规方法进行菌株分离,用ATB半自动微生物分析仪进行菌株鉴定,K-B法做药敏试验,ESBLS应用双纸片协同实验法检测。结果:共分离大肠埃希氏菌253株,产ESBLS大肠埃希氏菌株122株,阳性率为44.3%,有上升趋势。ESBLs对青霉素类及喹诺酮类耐药率最高,对AMP和CIP耐药率分别为89%和75.8%,对IPM均敏感。结论:大肠埃希氏菌在临床标本中的检出率、耐药性均有所上升。ESBLs大肠埃希氏菌引起的感染,治疗应首选IPM或含β-内酰胺酶抑制剂的药物,FOX、AK和FEP刘ESBLs也有较好的作用。临床应根据药敏试验及细菌耐药酶的检测正确选用抗菌药物。
Objective Observing antibiotic resistance of E.coli to suggest the clinical medication.Methods E.coli were isolates and identified by ATB semiautomatic microoganism analyzer.Antibiotic susceptibility test was assayed by K-B test and ESBLs was determined by the confinmation method.Results 253 E.coli were isolated and 122 E.coli producing ESBLs were assayed,the positive incidence was 44.3%,The resistance rate of ESBLs producing strains to penicilliums and quinolones were highest,the resistance to AMP and CIP was 89% and 75.8% separately.All tested strains were susceptible to IPM.Conclusion The positive incidence and resistance rate of E.coli went up.To deal with the ESBLs producing strains,IPM or β-lactamase inhibitor were preferred,FOX,AK,FEP can be used.
