34βE12 CD10及SMMHC在乳腺良恶性病变鉴别诊断中的意义

摘要 目的探讨乳腺肌上皮细胞标记物平滑肌肌球蛋白重链(SMMHC)、34βE12、CD10在乳腺良、恶性上皮性病变鉴别诊断中的意义。方法用免疫组织化学方法检测肌上皮细胞标记物SMMHC、34βE12及CD10在60例乳腺良、恶性病变中的表达,分析其与乳腺良、恶性上皮病变的关系及意义。结果34βE12、CD10和SMMHC在乳腺良性病变中的表达率均为100%;在乳腺癌中的表达率分别为26%、30%、35%;三者阳性表达率及其强度在良性病变与癌组之间差异有统计学意义(P<0.05);并且在乳腺癌中,SMMHC与34βE12和CD10相比较差异有统计学意义(P<0.05)。结论SMMHC、34βE12及CD10可作为乳腺病变鉴别诊断中肌上皮细胞的标记物,但SMMHC在标记乳腺肌上皮细胞时特异性及敏感性均优于34βE12和CD10。 Objective To investigate the significance of breast myoepithe!ial cell markers 34βE12, CD10 and SMMHC in the differential diagnosis of benign and malignant breast lesions. Methods Expressions of 34βE12, CD10 and SMMHC were detected by immunohistochemical methods in 60 cases of benign and malignant breast lesions. The significance of the relationship among myoepithelial celt markers 34βE12, CD10 and SMMHC in benign and malignant breast lesions were analyzed. Results The rates of positive expression were all 100% for 34βE12, CD10 and SMMHC in benign breast lesions, and 26% for 34βE12, 30% for CD10, and 35% for SMMHC in breast cancer, suggesting a significant difference between benign lesion and malignancy(P〈0.05). Moreover, the level of SMMHC expression in breast cancer was significantly different from those of 34βE12 and CD10 (P〈0.05). Conclusion SMMHC, 34βE12 and CD10 can be useful markers for differential diagnosis between benign and malignant breast lesions. SMMHC may be more specific and sensitive as a marker for breast myoepithelial cells than 34βE12 and CD10.
