摘要
目的 探讨多普勒组织成像(DTI)技术对室性早搏起搏点定位的诊断价值,并观察无水酒精直接作用于异位起搏部位的药物效果。方法 应用DTI速度图模式与加速度图模式与心电图同步,观察实验猪频发室性早搏和按需起搏心律的心肌显像特征,并在DTI引导下直接对室性紊乱心律起搏部位注入无水酒精。结果①DTI与心电图同步能显示实验性室性早搏与起搏心律发生的具体部位;②DTI的彩色编码反映了心室某异位起搏点的色彩、运动速度均高于正常心室肌;③DTI显示室性异位起搏点重复性好;④频发室性早搏起源部位注入无水酒精后早搏消失。结论 应用DTI超声新技术对室性早搏起搏点与室性起搏心律定位诊断确切、可靠,无水酒精对室性心律紊乱异位兴奋点有抑制作用。
to assess the value of Doppler Tissue Image (DTI) on the location of the abnormal exciting premature ventricular arrhythmias, to observe the therapeutic effect of anhydrous ethanol directly on the abnormal exciting point. Methods DTI meanwhile ECG to observe the characteristics of the myocardial image for the abnormal exciting premature ventricular arrhythmias and demand pacing rhythm in the animal. Anhydrous ethanol was directly injected into the exciting spot with DTI guiding. Results (1)DTI synchnonous ECG can show location of the abnormal exciting premature ventricular arrhythmias and pacing rhythm;(2)The velocity and color code of the abnormal exciting point was higher than the normal ventricle myocardium, and could be displayed by DTI color code;(3)Reproductibility test on the ventricular abnormal exciting by DTI were well expressd; (4)After injecting anhydrous ethanol, ventricular premature beat disappeared. Conclusion DTI behaves as a new sensitive and valid method to assess the location of the abnormal exciting premature ventricular arrhythmias. The abnormal exciting point of ventricular arrhythmias can be restrained by anhydrous ethanol.
