多巴酚丁胺静脉滴注引起心肌缺血与心律失常的临床观察

Clinical obsevation of dobutamin infusion to induce myocardial ischemia and arrhythemias

本文报道31例冠心病，30例正常对照组，用不同剂量多巴酚丁胺（DB）静滴前后心电图观察心肌缺血、心律失常，心率和血压的改变。结果DB能加重心肌缺血，与心律失常，且剂量越大，心肌缺血反应越明显，心脏病情严重者，仅5μg/kg/min；即可加重心肌缺血。本文提示DB仅用于常规抗心衰无效者，且剂量要个体化。

In this article, 31 cases cororary heart disease and compared with 30 cases normal persons is reported.The both groups, ECG is observated before, during, after dobutamine infusion with separate and ischemia ST - T change, arrhythmia , HR, BP and angina is monitored. The results shows dobutamine can increased myocardia ischemia and precipitated arrhythemia. As dose of dobutamine infusion is larger or heart condition is serious, even in lower dose,only 5 μg/kg/min, dobutamine infusion can induce myocar dial ischemia and severe ventricular arrhythemia. The article consider that dobutamine is used only after the standard treatment of congestive heart failure is not effect, The dose shouldbe used separately according to disease condition, < 5μg/ kg/min might be a due dose.
