10公斤以下婴幼儿体外循环114例体会

Experiences of cardipulmonary Bypass Management of 114 Children with Body Weight under 10kg

本文报道了我院1992年1月到1996年12月10公斤以下婴幼儿体外循环114例。心肺转流时间23~313分钟（平均78．4±42．0分钟），主动脉阻断时间0~88分钟（平均42．0±15．7分钟）。106例心脏自动复跳，自动复跳率95．5％。我们的体会是：选用膜式级合器，尽量减少预充量，使用抑肽酶及糖皮质激素，足够的流量，良好的心脑肺保护等是10公斤以下婴幼儿体外循环心脏手术成功的保证。

From January 1992 to December 1996 114 infants with congenital heart disease and body weight under 10kg underwent cardiopulmonary bypass(CPS) and open heart surgery.CPB time ranged form 23min to 313min (mean 78. 4 ± 42. 0 min). Aortic cross clamping time ranged form O min to 88min (mean 42.3±15.7min). 106 hearts returned spontaneously ,occupied 95.5% of the 111cases taken arotic cross clamping.Our experiences are:adopting membrance oxygenator, minimizing perfute volume,applying aprotinin and glucocorticoid,,good heart,brain andlung rpotectin are important factors to successful CPB opera tion for children with body weight under 10kg.
