高龄无症状人群大肠癌癌前疾病筛检方法的比较

A comparative study for screenimg program of colorectal adenoma in asymptomatic population aged over 50 years

参加普查的50岁以上无症状人群419名,其中208人同时接受了粪便隐血(OB),粪便微量白蛋白(MA),粪便大肠癌抗原(Sc)和纤维结肠镜检查。普查发现43例大肠腺瘤,腺瘤发生率20.67％。几种大肠肿瘤普查方案中OB,MA.Sc,OB+MA,OB+Sc,MA+Sc,OB+MA+Sc和直接肠镜检查,以直接肠镜检查的效—价比最佳。 结论:高龄人群的大肠肿瘤普查方案应采用直接肠镜检查,而年轻者则以采用OB+MA方案更为恰当。

208 asymptomatic individuals aged over 50 years were screened with several different programs simultanously; OB (Fecal Occult Blood program), MA (Fecal Micro Albumin program), Sc (Fecal colonic Carcinoma Antigen program), OB+MA, OB+Sc, MA+Sc, OB + MA + Sc and colonoscopy without pre-fecal screening tests (CS).43 cases of colorectal adenoma were detected and the incidence was 20.6%. In terms of the cost-effect, CS was indicated as the best program in all of the programs for aged men.Couclution: Colonoscopy should be recommended as the first choice for the screening of colorecral adenoma in asymptomatic population over 50 years old, whereas OB+MA is a more suitable program for the younger population.
