脑梗塞致闭锁综合征的护理——附病例报告

Care of Locked - in Syndrome Caused by Cerebral infarction

闭锁综合征(Locked-in,syndrome)是一种特殊类型的意识状态。病人表现意识清楚,四肢瘫痪,不能讲话,可借助瞬目闭眼动作和眼球垂直活动来表达思维。由于病人不能吞咽及咳嗽排痰,多数合并肺炎死亡。通过对一例由于脑梗塞致闭锁综合征的患者的护理,探讨如何加强呼吸道护理及已形成的褥疮的护理,使感染得到很好的控制,降低合并症的发生。

A case of Locked-in syndrome caused by acute pontine infaction is reported. The clinical manifestation include verbal capability deficits, paralysis of the limbs caused by basilar artery occlusion. In this condition, the patient can communicate only by eyes and eyelips movements.Key points in following nursing process :(1) Taking care of the airway: trcheotomy, sputium aspiration drainage, spray inhalation.(2) Care of skin: intensive nutrition, dressing changing with special drug.(3) Infusion care.(4) Psychological nursing.
