64例心脏粘液瘤外科治疗分析

ANALYSIS OF 64 CASES OF CARDIAC MYXOMA TREATMENT

本文报道64例心脏粘液瘤的手术治疗。术中发现粘液瘤在左心房56例,右心房7例;左心室1例。左心房粘液瘤中有2例为恶性粘液瘤。64例肿瘤均在体外循环直视下切除。术前确诊依靠超声心动图和多普勒检查,手术后死亡3例,死亡率为4.7%。

Sixty-four cases of cardiac myxoma were operated upon from January 1978 to October 1989, among them fifty-six were left atrial,two of the left atrial myxomas were malignant; seven were right atrial, one was left ventricular.The diagnosis was confirmed by echocardiography. The myxoma, pedicle together with its attachment(the atrial septum or the endothelium) were excised with the aid of cardio-pulmonary bypass. The cardiac cavities were irrigated with saline to remove all of the fragments.There were 3 early postoperative deaths with an overall mortality rate of 4.7%. Postoperative follow-up revealed that all patients are living well except one case of malignant left atrial myxoma died of tumor recurrence in the heart and extensive metastasis.
